of Topical DHEA vs Oral DHEA Supplementation
DHEA taken orally is first processed by the
liver where up to 80% is eliminated. The remaining DHEA is converted to a
larger DHEA molecule called DHEA-S which is known as a stored version of DHEA.
This larger molecule is too big to cross the blood brain barrier so its effects
cannot be felt in the Central Nervous System (which includes the brain). When
DHEA is applied to the skin (topical) it is not converted to DHEA-S, but is
distributed via the blood stream and used throughout the whole body and brain.
Because topical DHEA does not go through the liver first, lower doses are
needed to be effective. Typical dosage is approximately 20% of an oral dose.
Functions of DHEA and DHEA-S
The
typical ratio of DHEA to DHEA-S in our bodies is 2%/98%. DHEA-S levels do not
vary day to day unlike DHEA and cortisol which have strong diurnal rhythms.
DHEA tends to have a shorter term pulsatile effect whereas DHEA-S works on a
longer, genomic timeline. DHEA and DHEA-S have separate and overlapping
functions. Both hormones modulate neurotransmitter synthesis and release,
immunity and inflammation, endothelial and cognitive function, and neurogenesis
and neuronal survival.   The body has many feedback loops to control hormone levels. Not so with DHEA: DHEA levels decline with age and, as DHEA levels decrease, it’s lack is felt throughout the body and brain. Recommendation: As with all bioidentical hormones, regular, annual saliva testing of DHEA will support use. DHEA-S is a polar molecule and not present in saliva but can be identified in serum.
